Optum360 - NorthWell Health

As part of Northwell Health’s commitment to ensure the health and safety of its workforce and patients, and in compliance with New York State Department of Health regulations, Optum360 employees working in Northwell Health clinical facilities are required to successfully complete Tuberculosis (TB) screening, immunizations and an Annual Health Assessment.  Refer to your email from MAEN_notification@medgate.com to see which requirements you are due to complete.

Upon your arrival:

1. Identify that yourself as an Optum360 employee.

2. Provide the following information:

· Your legal name as it is listed on your Driver’s License

· The facility in which you work

3. Request the required services listed in your MAEN notification email.
